What is human in the loop?

Definition

Human in the loop is a design in which a person takes part at defined points of an automated process, by approving an action, taking over a task, or reviewing results afterward. For a voice agent, it is how a call can reach a human.

How human in the loop works

The design question is where the person sits. Three patterns cover most voice deployments.

The first is takeover. The agent handles the call until a rule or the caller sends it to a person: the caller asks for one, the agent fails twice to understand, or the request is on a list of things it may not do. The agent then transfers the call, ideally with a summary so the caller does not repeat themselves. See warm transfer.

The second is approval. The agent prepares an action but a person authorizes it before it takes effect, for example a refund over a set amount. On a live call this means the caller waits, so many teams complete the call and finish the approval afterward by callback or message.

The third is review. The call completes without a person, and humans sample recordings, check transcripts and flag failures. This does not protect the call in progress, but it is how errors are found and the instructions improved.

Why it matters for voice agents

A caller on the phone cannot see the agent’s confidence and has no screen to correct it. A person in the loop is the safety valve for calls the agent should not finish, such as distress, disputes, medical or legal questions, and anything that cannot be undone. Deciding where those lines are, and writing them as explicit rules, is more reliable than hoping the agent notices.

The human step has costs of its own. A transfer to a queue with no one available is worse than no transfer, so the destination, hours and fallback need testing. Each handoff also adds to the escalation rate, which is useful for judging whether the agent is doing its job. A rate near zero can mean the agent is handling the calls or that it is never letting go of them.

Example

An illustrative policy for a billing line. The agent can look up a balance and set up a payment plan under a set amount. A dispute, a request to speak to a person, or two failed attempts to capture an account number triggers a warm transfer with a short summary. A reviewer listens to a sample of finished calls each week.
